The Licensed Nursing Assistant performs personal care and designated health-related services for Home Health and Hospice patients in their homes under the supervision of a Registered Nurse. This includes assisting with Activities of Daily Living, observing and reporting changes in patient condition, and maintaining professional standards while interacting with families and the healthcare team.
Candidates must possess a High School diploma or GED accompanied by a current New Hampshire Licensed Nursing Assistant (LNA) license. A minimum of one year of recent experience working as a Licensed Nursing Assistant is also required for this position.

The Granite VNA Home Health and Hospice Licensed Nursing Assistant (LNA) performs personal care and designated health-related services for Home Health and Hospice patients in their homes primarily working in our service area Facilities. The Home Health and Hospice LNA interacts with client’s family members and other healthcare team members.
Position Overview Reporting to the Paraprofessional Administrative Manager, the Granite VNA Home Health & Hospice Aide performs personal care and designated health-related services for the care and well-being of assigned clients under the supervision of a Registered Nurse. The Home Health & Hospice Aide interacts with client’s family members, facility staff and other healthcare team members while maintaining professional standards.
High school diploma/GED, accompanied by current New Hampshire LNA license One year of recent experience as a Licensed Nursing Assistant
