The Hospice Community Registered Nurse delivers age-appropriate care to patients in home and facility settings, utilizing nursing best practices. Responsibilities include developing care plans, documenting patient assessments, and coordinating with interdisciplinary teams.
Candidates must be graduates of an accredited nursing school with a current New Hampshire nursing license and have three to four years of recent nursing experience. The role also requires effective communication and supervision of nursing staff.

As a Hospice Community RN, you’ll provide compassionate, high-quality nursing care to patients in their homes and facility settings throughout the Granite VNA service area. You’ll partner closely with patients, families, providers, and the interdisciplinary team to develop individualized plans of care that support comfort, dignity, and quality of life.
What You’ll Do Assess patient needs and develop individualized care plans Provide hands-on nursing care, education, and emotional support to patients and families Collaborate with physicians and interdisciplinary team members to coordinate care Document patient progress and communicate changes in condition Participate in care conferences, team initiatives, and ongoing education Supervise and support LPNs and LNAs assigned to patient care Travel throughout the service area to provide in-home patient visits
Graduate of an accredited school of nursing, accompanied by current New Hampshire nursing license, or compact state license through the Nurse Licensure Compact (NLC). One to two years of recent general medical/surgical or skilled/acute care nursing experience.
