The Occupational Therapist develops and implements appropriate plans of care and discharge goals based on thorough patient assessments, ensuring accurate documentation of progress and patient/caregiver education. They must effectively coordinate the patient's plan of care with the interdisciplinary team and other health practitioners.
Candidates must be a graduate of an approved occupational therapy program and possess a current New Hampshire occupational therapy license. A minimum of one year of occupational therapy experience is required for this full-time position.

Position Overview Reporting to the Home Care Director, the Granite VNA Occupational Therapist (OT) delivers age appropriate care to the patient in the home setting utilizing standard therapy practice: assessment, planning, intervention, implementation and evaluation within scope of practice defined by the NH Occupational Therapy Board. OTs will effectively interact with patients, caregivers and other healthcare team members while maintaining all standards of professional therapy. OTs will supervise Certified Occupational Therapist Assistants assigned to patients.
Minimum Qualifications and Requirements Graduate of an approved occupational therapy program, accompanied by current New Hampshire occupational therapy license. One year of occupational therapy experience.
