The role involves providing skilled physical therapy services to adult and geriatric patients in a skilled nursing and post-acute rehabilitation setting, focusing on evaluation and treatment to improve functional independence. Responsibilities include developing plans of care (PT) or implementing treatment programs (PTA), monitoring patient response, and maintaining accurate documentation.
Candidates must be a graduate of an accredited Physical Therapy or Physical Therapy Assistant program and hold the current PT or PTA license for the applicable state. Experience in skilled nursing, long-term care, or post-acute rehabilitation is preferred, alongside strong clinical reasoning and organizational skills.

Physical Therapist (PT) or Physical Therapist Assistant (PTA) – Panora, IA Blue Stone Therapy is seeking a Physical Therapist or Physical Therapist Assistant to provide rehabilitation services to adult and geriatric patients at Panora Specialty Care in Panora, IA. This role focuses on evaluating and treating adult and geriatric patients in a skilled nursing and post-acute rehabilitation setting. The PT or PTA will work as part of an interdisciplinary rehabilitation team to improve mobility, strength, balance, endurance, pain management, and overall functional independence.
