This role provides non-clinical administrative support for the care management function, contributing to integrated care delivery across the continuum. Essential duties include facilitating administrative tasks such as case assignment, scheduling, correspondence processing, and data entry for care management services.
Candidates must have at least one year of administrative support experience in healthcare or equivalent education, along with strong skills in attention to detail, problem-solving, time management, and organization. Proficiency in Microsoft Office is required, and a Certified Medical Assistant certification is preferred.
