The Nurse Practitioner will conduct 5-7 home visits daily for patients with complex care needs and perform comprehensive health evaluations. They will also document patient information accurately and educate patients and their families on seeking medical care.
Candidates must have a minimum of 2-3 years of home health care or home visit experience and must be a graduate of an accredited Nurse Practitioner program. Current Board Certification as a Nurse Practitioner is also required.
