Case Managers coordinate client-focused treatment and services, embracing a trauma-informed approach and using motivational interviewing to help individuals achieve wellness. Responsibilities include assessment, coordination of care, service plan development, resource development, and medication support and management within a community-based setting.
Required qualifications include being a Qualified Mental Health Professional (QMHP) with a Bachelor’s Degree in a relevant Human Services field, specialized training or one year of experience working with persons with mental illness, and a valid unrestricted Driver’s License with automobile accessibility. Preferred qualifications include a Master's Degree and relevant state licensure.

Our case managers coordinate client-focused treatment and services, embrace a trauma-informed approach, and leverage motivational interviewing techniques with the goal of helping people achieve wellness. They provide population and age-appropriate case management services to the individuals we serve, including assessment, coordination of care, service plan development, resource development, and medication support and management.
While this position is primarily community-based, it provides opportunities for some remote flexibility upon the successful completion of the training/onboarding period.
The starting pay for this position with Bachelors is $40,000. to $48,300. for unlicensed candidates, $49,300. - $63,700. for limited licensed candidates, and $51,000. - $66,000 annual salary for fully licensed candidates. With Masters $40,800. to $55,200. for unlicensed candidates, $52,700. to $68,300. for limited licensed candidates, and $56,100. - $72,900. annual salary for fully licensed candidates and is based on non-discriminatory factors such as skills and experience.
