Job description
The Center for Human Development, (CHD) is seeking Direct Care Staff to join our team at our Iris residential program in New Hartford, CT. Our direct care staff establish life-changing relationships with our participants and provide opportunities to optimize their independence, rehabilitation and recovery.
Our Iris group home program is dedicated to servicing individuals with a high level of chronic psychiatric conditions, enabling them to live a life of high quality.
The program specializes in supporting clients who have experienced challenges with community living by ensuring the individual needs are met through each client’s uniquely created treatment plan.
To ensure a high level of care, Iris House supports clients by providing 24/7 on-site staffing.
Iris House takes a team-based approach to the all-around care of the clients.
While clients may initially struggle to shed behaviors learned from years of self-preservation, we provide the structure, stability, care and attention needed to let go of their diagnoses to reveal the person underneath.
Your role as a Direct Care Staff
The Direct Care Staff works within the Agency’s team model, establishing open communication, support, and accountability among co-workers. Providing a supportive residential environment for individuals who are challenged by mental health and substance abuse histories in a community-based group home. Our goal is to provide an environment that encourages program participants to grow and to live as independently as possible. Some additional responsibilities include, but are not limited to:
Daily activities include providing direct services to program clients living in a group home setting in the community
Supporting clients' activities of daily living and monitoring clients' self-administer medication
Advocating for individuals' within various community settings, and providing opportunities to optimize their independence, rehabilitation and recovery
Develop professional and supportive relationships with clients and provide informal counseling and guidance with a person-centered focus

Full-time
1st shift, 6
00am-2:00pm.
Days on: Sunday, Monday, Tuesday, Wednesday, Saturday.
Days off: Thursday, Friday.
3rd shift, 10
00pm-6:00am.
Days on: Sunday, Monday, Tuesday, Friday, Saturday.
Days off: Wednesday, Thursday.

Those having a passion for helping others with kind and a positive approach make for an ideal candidate for the Direct Care role. Additionally, having strong interpersonal skills, empathy for others, and demonstrated commitment to serving those with significant needs, ability to work well within a team model and independently. You will also need to bring good written and verbal skills, and a strong level of compassion to meet the needs of the individuals served.
