The Medical Social Worker is responsible for determining client needs and connecting them with necessary support services, such as counseling, meal assistance, and financial aid for medications or living expenses. This role also involves integrating and coordinating social service and discharge planning activities within the hospital/LTC system through effective communication.
Candidates must possess either an LSW or MSW license and have a minimum of two years of experience in Social Work, maintaining state licensure. Essential functions include providing services across all patient populations and adhering to all hospital safety and professional conduct standards.

The Medical Social Worker determines clients’ needs and helps them get support services, such as counseling, meal services, and financial assistance for medications and/or living needs. Maintains performance improvement activities within the department.
Lsw or msw.
Minimal two years experience in Social Work.
Licensed in state LSW or MSW.
Assures that social service/discharge planning activities are integrated and coordinated with the hospital/LTC system by effective communication at all levels.
Provides services to specific patient population, including neonate, pediatrics, adolescent, geriatric and general population.
Is knowledgeable of and utilizes appropriate community resources.
Attends all required safety training programs and can describe his/her responsibilities related to general safety, department/service safety, and specific job-related hazards.
Maintains standards for licensing in the social work field.
Follows the hospital exposure control plans/bloodborne and airborne pathogens.
Demonstrates respect and regard for the dignity of all patients, families, visitors, and fellow employees to ensure a professional, responsible, and courteous environment.
Promotes effective working relations and works effectively as part of a department/unit team inter- and intradepartmentally to facilitate the department’s/unit’s ability to meet its goals and objectives.

Appearance is professional, neat, clean, and appropriate for the work environment. Wears proper photo identification badge while on duty.
Maintains confidentiality of all hospital and patient information at all times, as observed by peers and management.
Attends monthly staff meetings and participates in monthly rounding process with manager to assure effective and timely two-way communications.
Participates in and completes required department specific education and training programs and can describe his or her responsibilities related to department safety and specific job-related hazards.
Follows safety and security policies including exposure control plans/blood borne and airborne pathogens.
