The Housing Case Manager provides supportive case management services and care coordination to a caseload of twenty to twenty-five formerly chronically homeless individuals and families with co-occurring conditions. Primary duties include conducting a minimum of one monthly home visit per client to provide services, supportive counseling, and referrals.
A Bachelor's degree in social work, psychology, counseling, or a related field, or a minimum of three years of relevant experience working with individuals experiencing homelessness, mental health issues, and/or substance use is required. An (L)MSW is preferred.
$24/hr - $31/hr commensurate with experience
We have three comprehensive healthcare plans to choose from based on your priorities and budget. Housing Works covers most of the plan; you pay a portion, based on your salary. Staff begins accruing PTO immediately for a total of up to 30 days earned in the first year. We offer employees an educational benefit. This money is available for tuition loan reimbursement, tuition costs, and text books.
The SHIP Housing Case Manager (HCM) provides supportive Case Management services and care coordination to a caseload of between twenty (20) and twenty-five (25) formerly chronically homeless individuals and families with co-ocuring conditions. The Housing Case Manager works closely with their clients in the clients’ homes, in the community, and the office, helping to ensure housing stability by promoting independent living skills, financial management, and health and wellness.
• The HCM successfully conducts a minimum of one monthly home visit to each client on their caseload, and provides case management services, supportive counseling, and referrals.
• The HCM maintains client records as per funder and regulatory requirements, as well as Housing Works standards.
