The Licensed Practical Nurse will provide direct nursing care, administer medications accurately, and develop, implement, and evaluate individualized care plans for residents. Responsibilities also include maintaining medical records, assisting with daily living activities, responding to emergencies, and collaborating with the interdisciplinary team.
Candidates must possess a current LPN license in Pennsylvania and hold CPR certification along with a valid PA driver's license. Experience with individuals with intellectual and developmental disabilities is preferred, and the role requires the physical ability to lift/move 25–50 lbs and assist with transfers.
