The role involves overseeing and managing resident care delivery from pre-admission through post-admission, ensuring appropriate care coordination and utilization of services. Key duties include collaborating with teams to develop care plans, monitoring resident progress, facilitating smooth transitions of care, and advocating for residents and families.
Candidates must have successfully graduated from an approved LVN program and possess a current, unencumbered LVN license in the state. Required attributes include the ability to make quick judgments, work independently, and possess knowledge of relevant nursing practices, regulations, and guidelines pertaining to long-term care.

As a Full-Time Case Manager/MDS (RN) (LPN/LVN), you will oversee and manage the delivery of care for residents from pre-admission through post-admission. You will play a pivotal role in ensuring residents and their families receive appropriate care coordination and utilization of services to ensure the highest quality care.
