The Case Manager acts as central support within the interdisciplinary team by conducting assessments, connecting members with community resources, and coordinating care both internally and externally. Responsibilities include developing service plans, supporting treatment planning, advocacy, medication management, crisis intervention, and maintaining accurate documentation.
Applicants must be a minimum of 21 years old and possess a Bachelor's degree in a behavioral health field along with one year of relevant experience in behavioral health. A valid Arizona driver's license, motor vehicle clearance, and the ability to obtain a fingerprint clearance card are also mandatory.

We are seeking a Case Manager to join our Metro Outpatient Clinic. This location provides mental health treatments, including medication monitoring and assistance for persons with addictions.
The Case Manager will work in-office Monday-Friday, standard business hours (no evenings, weekends or holidays).
The Case Manager serves as a central support within the interdisciplinary team. This staff member conducts assessments, assists members with resources within the community and provides coordination of care inside and outside of the agency.
