The role focuses on providing intensive, in-home interventions to support youth and families, working alongside a licensed clinician to build family stability and reduce crisis situations. Responsibilities also include coordinating care, engaging community partners, and participating in a crisis response rotation.
Candidates must possess a Bachelor's degree in a Human Service field of study and have one year of post-degree experience working with at-risk youth populations. Essential requirements include willingness to provide community/home-based care, participation in crisis rotation, and proficiency with EHR systems.
