The coordinator will assess residents' psychosocial and spiritual needs, coordinating admissions, discharges, and ensuring care continuity while providing crisis intervention and emotional support to residents and families. Responsibilities also include maintaining accurate documentation, completing social work assessments like the MDS, and connecting residents with community resources across Nebraska.
Candidates must possess empathy, respect, and emotional maturity for serving older adults, along with strong communication, organization, and teamwork skills to support care planning. A high school diploma or equivalent is required, along with the completion of the Nebraska Health Care Association Social Service Designee training.
