The Care Coordinator will manage follow-up care for patients transitioning from hospitalization or emergency room visits, focusing on medication reconciliation and discharge planning. This role also involves supporting preventive health initiatives and educating patients on chronic care management.
Candidates must be a licensed Registered Nurse (RN) or Licensed Practical Nurse (LPN) in Nebraska, a graduate of an accredited nursing school, and hold current BLS certification. Prior care coordination experience is preferred, alongside strong communication abilities.
