The Physical Therapist provides physical therapy services, including clinical evaluation and treatment, to program participants in acute, restorative, or maintenance settings as required. This role involves integrating the physical therapy treatment plan into the Interdisciplinary Team's overall plan of care and assisting with durable medical equipment management.
Candidates must be graduates of an American Physical Therapy Association-approved curriculum and currently licensed as a Physical Therapist in the state of practice, requiring a minimum of one year of experience working with frail or elderly populations. Additional requirements include clearing a background check, possessing a valid driver’s license, and maintaining current medical clearance for communicable diseases and immunizations.

The Physical Therapist under the supervision of the Center Manager, provides physical therapy services to program participants. Provides clinical physical therapy evaluation and treatment for program participants, on acute, restorative or maintenance levels as needed by Day Center, Nursing Home or in-home setting. Integrates the physical therapy treatment plan into the Interdisciplinary Team plan of care. Assists in the ordering, inventory, distribution and maintenance of durable medical equipment for participants.
