The Medication History Technician assists pharmacy, nursing, and medical staff by obtaining accurate home medication lists from newly admitted patients through interviews, chart review, and external calls. This role involves accurately processing and entering this medication and allergy information into computer systems, resolving discrepancies with providers, and collecting data for quality reporting.
Candidates must possess Virginia Board of Pharmacy technician licensure or a Licensed Practical Nurse license in Virginia, along with a minimum of two years of experience as a Pharmacy Technician or LPN. Preferred qualifications include PTCB certification and two years of experience in a retail pharmacy, physician office, or outpatient setting, alongside strong math and interpersonal skills.

A minimum of two years of Pharmacy Technician or LPN experience required. A minimum of two years of experience in a retail pharmacy environment, physician office or outpatient/ambulatory environment is preferred. Strong mathematics and interpersonal skills required. Ability to work with computers. Ability to work in a versatile environment and manage multi-task demands. Capacity to cope with difficult situations. Ability to tolerate irregular hours including evenings, nights, and weekends.
