Act as a liaison for Medicaid members to ensure access to appropriate care through home and social assessments. Collaborate with clinical teams to develop care plans and support members in managing health, mental health, and substance use treatment.
Requires a High School Diploma/GED, valid US driver's license, and residency in New Mexico for at least 2 years. Candidates must have PC proficiency and the ability to travel locally up to 50% of the time.
Create a positive experience and relationship with the members Practice cultural sensitivity and cultural competence in daily care Learn and listen to members’ needs and barriers to help promote self-advocating Work with care coordinator to develop a care plan and set goals for member care Collaborating with clinical team of social aspects that might impact treatment plan Proactively engage the members to manage their own health and healthcare Support the members to improve their wellbeing by staying out of the hospital, and attend regular visits to their primary physician As needed, help the member engage with mental health and substance use treatment Support the member to ensure pick-up of their prescriptions Provide member education and health literacy on community resources and benefits to encourage self sufficiency Conduct post discharge activities from hospital facility and support connection to social services Support members to engage in work or volunteer activities, if desired, and develop stronger social support through deeper connections with friends, family, and their community Partner with care team (community, providers, internal staff) Knowledge and continued learning of community cultures and values Conduct Health Risk Assessments (HRA) You’ll be rewarded and recognized for your performance in an environment that will challenge you and give you clear directions on what it takes to succeed in your role as well as provide development for other roles you may be interested in.
High School Diploma/GED (or higher) Intermediate level of proficiency navigating a PC to open applications, send emails, and conduct data entry Intermediate level of knowledge of culture and values of the community Intermediate level of familiarity with the resources available in the community Access to a designated workspace inside the home with access to high - speed internet availability Currently reside in New Mexico Must have lived in New Mexico area for 2 years Ability to travel locally up to 50%25 of the time Access to reliable transportation and valid US driver’s license
Bachelor’s Degree (or higher) in a health-related field Licensed Practical Nurse (LPN), Certified Nursing Assistant/Home Health Aide, Certified Medical Assistant CHR/CHW Certification 2+ years of field-based experience Previous care management experience Experience working in Managed Care Knowledge of Medicaid and / or Medicare population Knowledge and/or experience with behavioral health or substance use disorders *All Telecommuters will be required to adhere to UnitedHealth Group’s Telecommuter Policy.
