The Case Manager acts as a central support within the interdisciplinary team, primarily serving adults by conducting assessments and coordinating care both inside and outside the agency. Key duties include completing intake screenings, annual behavioral health assessments, facilitating various team meetings (CFTs, ARTs), leading groups, and identifying gaps in member care.
Applicants must be a minimum of 21 years old and possess a Bachelor's degree in a behavioral health field along with one year of relevant experience, though a combination of education and experience totaling two years is acceptable. A valid Arizona driver's license and fingerprint clearance from the Arizona Department of Public Safety (or eligibility within eight weeks of hire) are mandatory.
