The Case Manager – Hospital Liaison acts as the main coordinator between hospitals, Managed Care Organizations (MCOs), and Rosecrance Crisis programs, ensuring timely communication on client status and discharge planning. This role also monitors documentation to maintain compliance with program fidelities, payer requirements, and regulatory standards across all Illinois regions.
Candidates must possess a Bachelor's degree in a related field or equivalent experience and qualify minimally as a Mental Health Professional (MHP) per Illinois Rule 132. Essential qualifications include strong communication skills, proficiency with EHR systems, excellent organizational abilities, and a belief in the organization's mission.

The Case Manager – Hospital Liaison serves as a primary point of coordination between hospitals, Managed Care Organizations (MCOs), and Rosecrance Crisis programs. This role facilitates timely communication regarding client status, crisis outcomes, and discharge planning to ensure continuity of care. The position also monitors and manages compliance-related documentation to ensure services are delivered in accordance with program fidelities, payer requirements, and regulatory standards. The Case Manager – Hospital Liaison works collaboratively with leadership and crisis program teams across all Illinois regions to support effective transitions of care and timely completion of required tasks.
1. Work scheduled hours and any additional hours deemed necessary by leadership. Some variation in scheduled hours may be required to meet company needs.
2. Serve as a liaison between hospitals, MCOs, and Rosecrance crisis programs to support card coordination and continuity of services.
3. Participate in consultations with hospital staff and crisis program teams to provide updates on client status, treatment progress, and crisis outcomes.
4. Coordinate discharge planning efforts to ensure appropriate level of care placement and timely linkage to Rosecrance or community-based services.
5. Complete utilization revies and provide necessary clinical and administrative information to insurance providers in accordance with payer requirements.
6. Monitor crisis program documentation to ensure compliance with program fidelities, regulatory standards, and internal policies.
7. Communicate documentation needs, deadlines, and compliance concerns to leadership and crisis program staff in a clear and timely manner.
8. Collaborate with crisis program staff across all Illinois regions to support effective case management and task completion.
9. Serve as a professional representative of Rosecrance, demonstration positive guest relations and strong customer service with clients, families, and external partners.
10. Understand and comply with Rosecrance principles, policies, and procedures.
11. Perform all responsibilities in alignment with the mission, values, vision, and expectations of Rosecrance.
12. Assume other related duties as assigned by leadership to support program operations and continuity of care.
