This direct care role involves assessing and monitoring client needs, developing individualized service plans, and connecting residents with necessary community services and resources to support their recovery and independence. Additional duties include coordinating appointments, managing program enrollment, and developing transition plans for discharge.
Candidates must possess a Bachelor's degree along with one year of positively referenced relevant work experience, or an additional year of experience in lieu of the degree. A valid driver's license with three points or less and access to an insured vehicle are mandatory, along with availability for varied shifts including days, nights, weekends, and holidays.
The Adult Residential Treatment Center [https://www.sheppardpratt.org/care-finder/residential-mental-health-treatment-for-adults-adult-rtc/] (RTC) provides services for persons ages 18 and older with serious mental illness, including individuals with co-occurring substance use disorders or development disabilities. Our teams craft individualized plans for the residents to reach the highest level of recovery possible.

You will assess and monitor client needs, develop service plans, connect clients to community services and resources, and advocate for your clients as necessary.

Pay for this position is a flat rate of $20.34/hr.
