The Case Manager provides case management and support to Partial Care clients, facilitates groups, and documents according to regulatory standards. They carry a caseload and function as part of the Partial Care team, maintaining contacts with family members and community providers.
A Bachelor's degree in a human services field is required for this position. The role involves working with severely and persistently mentally ill individuals, who may also have substance abuse issues.
