The Admissions RN will be responsible for facilitating patient admissions to the hospice program by visiting patients, families, and healthcare professionals in various settings, including hospitals. This role also involves organizing discharge plans for hospitalized patients and ensuring timely and concise documentation in the Electronic Medical Record (EMR).
Candidates must possess an active New Jersey RN license and a valid NJ driver's license with required insurance coverage. A BSN degree and certifications like CHPN or OCN are preferred, along with a minimum of one year of relevant nursing experience, preferably in hospice or home care.
