The role involves providing assessment, supportive care, and education to prospective patients and families regarding hospice services, determining appropriateness, and facilitating admissions into the program. This includes acting as a liaison with hospitals, facilities, and physicians to coordinate transitions and complete comprehensive assessments.
Candidates must be a graduate of an accredited school of nursing, with a BSN preferred, and possess one to two years of experience in hospice, palliative, home health, oncology, or acute care settings. Required credentials include current Colorado RN licensure, CPR certification, a valid Colorado driver's license, and auto insurance.

Location: Colorado Springs, CO
Status: Part-time, 24 hours per week
Schedule: Sun/Mon/Fri 10a-7p or 11-8p
Schedule: Wed/Fri/Sat 10a-7p or 11a-8p
$46,425 - $54,550 annually (based on 24 hrs per week)
Supplemental pay: Based on position, schedule and/or availability: Paid Mileage/Shift Diffs/Stipends
Sixty-two and a half cents per mile – one of the highest in the industry!
