The Personal Care Assistant supports PACE Center participants by assisting with personal care, restorative rehabilitation, meals, and group/individual activities, while also ensuring safety during transportation. This role also involves contributing ideas for the activities program and reporting on participant care planning and assessment to relevant staff.
The position requires assisting participants with daily care needs, accompanying them during transportation, and potentially assisting supported housing participants with morning care. The candidate must actively contribute to the Day Center operations and participate in participant care planning through detailed reporting.
Assists PACE Center participants with personal care, restorative rehabilitation, meals, group and individual activities. Accompanies participants on transportation as needed to ensure safety. May be assigned to assist supported housing participants with morning care. Actively contributes to the operations of the Day Center by contributing ideas for the activities program and offering suggestions to improve daily operations. Contributes to participant care planning and assessment through reports to the Activities Coordinator, PACE Center Nurses and Rehabilitation staff.
