Requirements summary

Our home care caregivers provide professional and empathetic support across Northeast Ohio, divided into separate teams to better provide coverage.
This role will provide this support to the Greater Cleveland Area – Cleveland’s West, Downtown and East locations as far as Vermillion.
As an RN on this team, you will deliver the best possible care to patients in the comfort of their own homes.
In this role, you will provide assessments, complex treatments and education to help avoid readmissions to the acute care setting and ensure comfort.
This position offers a flexible schedule, allowing you to maintain a healthy work-life balance, all while changing the lives of your patients and their loved ones every day.
A caregiver in this position is working PRN and would need to be flexible from 12:00am - 12:00pm.
A caregiver who excels in this role will: Provide direct nursing care in accordance with established healthcare organization policies, procedures, and protocols.
Implement and monitor patient care plans.
Provide supervision to LPNs, Home Care Aides and families/caregivers involved in the care of the patient.
Perform accurate and complete admission assessments and reassessments within a specified time frame.
Assess patient for safety risks, abuse and neglect.
Serve as a primary coordinator of all disciplines for well-coordinated patient care.

Assess and document patients' pain using appropriate pain scale and manage patients' pain then.
Prioritize and document patient problems based on assessed needs and problems in collaboration with patients, family and healthcare team.
Maintain complete and up-to-date documentation and files at all times regarding patient condition and effectiveness and timeliness of care and interventions.
Escalate change in patient condition to patient’s physician as appropriate.
