The hospice medical social worker is responsible for conducting psychosocial evaluations and providing ongoing counseling to patients and their families throughout the end-of-life journey. This role involves delivering necessary emotional support to patients and families in accordance with the established plan of care.
Candidates must possess a heart for service and a passion for providing optimal patient care, requiring an MS degree in social work and current state licensure as a social worker. A minimum of two years of clinical work experience, preferably in healthcare or hospice, along with reliable transportation, is mandatory.

Ability to sit, stand, bend, move intermittently and lift at least 25lbs and bear the weight of an average adult effectively.
