The Behavioral Healthcare Provider develops and delivers personal care plans by conducting initial assessments, providing clinical intervention, making referrals, and offering treatment recommendations for participants and families. Responsibilities include completing mental health assessments in the EMR, consulting with team members, and collaborating with external providers to support participants at risk for inpatient psychiatric hospitalization.
A minimum of a Master’s degree in behavioral health, social work, or psychology is required, along with at least one year of experience working in an integrated care setting. Applicants must hold a current license as a Clinical Social Worker (LCSW), Psychologist (PsyD or PhD), Marriage Family Therapist (LMFT), or Professional Counselor (LPC), and must obtain CPR certification within the first 30 days if not already held.
