Provide primary and urgent care services to residents in sub-acute and long-term care facilities. Collaborate with the Medical Director and Center Leadership to improve health outcomes and achieve facility goals.
Must be a certified Nurse Practitioner (with a Master's degree) or a Physician Assistant (ARC-PA graduate) with valid state licensure and DEA. While new graduates are welcome, two years of experience with adult or geriatric populations is preferred.
