The Community Health Outreach Worker will conduct daily street outreach to engage homeless adults, connecting them to medical care, assisting with benefit applications, and providing referrals to psychiatric and social services. This role also involves providing health education, harm reduction interventions, and transporting clients to necessary appointments.
Candidates must possess a BA degree and/or equivalent experience in relevant fields like psychology or social work, along with two years of field experience working with homeless individuals or those with co-occurring disorders. A valid California Driver's License, clean DMV record, and use of a personal vehicle are required for client transportation.
Abode, one of the largest and effective nonprofits working to end homelessness in the Bay Area, is seeking a Community Health Outreach Worker for our programs in Alameda County.
The Community Health Outreach Worker will provide linkage, referral and follow along support to connect participants to primary care and ongoing medical and social services. This position will provide street outreach to engage homeless adults in medical care, apply for benefits, referral to psychiatric care, social services, and complete coordinated entry assessments.
