Job Summary

  • This position provides services and psychiatric support to Spero’s ACT (Assertive Community Treatment) team.
  • This multi-disciplinary team of eight individuals provides services to 40 clients experiencing serious and persistent mental illness.
  • The psychiatric care provider plays a vital role in guiding the ACT team and caring for its clients.
  • The ACT Provider will regularly meet with clients in the community and in their homes to determine their psychiatric treatment plan, assess symptoms, side effects, and treatment progress.
  • Beyond psychiatric needs, the provider monitors other health conditions, coordinates with primary care and specialists, and maintains communication with inpatient units during hospitalizations to ensure seamless continuity of care.
  • The ACT Provider serves as a fully integrated member of the multidisciplinary team, collaborating to provide feedback on client contact intensity, and educating non-medical staff about medications and health issues through regular trainings.
  • The ACT Provider participates in treatment planning meetings, daily team huddles, and emergency reviews, ensuring that care plans meet regulatory standards and that the team responds effectively to crises.

ACT Team Prescriber Expectations

  • Treatment planning with brief therapeutic interventions consistent with evidence-based practice and ethical guidelines.
  • Use a shared decision-making approach to medication education and treatment discussions.
  • Monitor non-psychiatric medical conditions and coordinate care with primary care providers and specialists.
  • Conduct a portion of psychiatric services in client homes and in the community with some psychiatric backup to the program outside of regular business hours and on weekends and holidays.

ACT Team Integration Requirements

  • The psychiatric provider must function as an integrated member of the multidisciplinary ACT team by:
  • Share overall clinical responsibility for treatment direction with the Team Lead and serve as medical director for multidisciplinary ACT team, taking the lead in all psychiatric treatments and monitoring all other health conditions and medications, while collaborating closely with nursing and support staff.
  • Adhering to the ACT model means meeting clients in person either in their homes or in the community as well as in the office, based on client preference. Local travel within the service area to meet with clients in person is necessary.

Required Knowledge, Skills, and Abilities

  • Background experience in leading psychiatric treatment of individuals diagnosed with serious and persistent mental illness with skill in identifying and assessing a wide range of patient needs with knowledge about local and regional community resources and how to access them.
  • Background experience with medication-assisted treatment (MAT).
  • Excellent interpersonal communication skills, dealing effectively with a variety of people and personalities; communicating clearly and effectively both orally and in writing.
  • Effective time management skills with the ability to work independently and as an effective team player, showing initiative and willingness to take charge of a situation.
  • Demonstrates thoroughness and organizational skills in the workplace and understands billing procedures and prior authorization processes for collecting reimbursement.
  • Shows proper etiquette when dealing with patients, and is respectful of other professionals on the ACT Team, and other methods of therapeutic intervention.
  • Displays flexibility to meet Spero’s and the patient’s needs in an environment with interruptions, including the ability to be available to provide psychiatric back up and support to the program outside of regular business hours which could include on weekends or holidays.
  • Understand and maintain the confidentiality of patient and organizational information.
  • Experienced in the use of standard computer applications, including but not limited to EMR systems, two-way live interactive video communication, email, and Microsoft TEAMS.

Education

  • Psychiatrist (MD/DO) licensed in Minnesota and board-certified or board-eligible in psychiatry (preferred) OR
  • Psychiatric APRN (PMHNP/CNS) with independent prescribing authority, psychiatric certification, and > 3 years unsupervised experience working with adults with serious and persistent mental illness

Work Environment: Office setting with approximately 30% community based work; traditional business hours
