The Clinical Pharmacy Specialist provides pharmacotherapy support to medical staff to optimize medication selection, dosing, and monitoring, participating in multidisciplinary rounds to enhance patient care. They serve as an evidence-based resource and are integral to Antimicrobial Stewardship, ensuring appropriate antimicrobial therapy usage.
Candidates must possess a Doctor of Pharmacy Degree (PharmD) and either one to two years of residency training or at least two years of clinical pharmacy work experience. A current Pharmacist License with the Texas State Board of Pharmacy is required, while specialized Board of Pharmacy (BPS) Certification is preferred.
The Clinical Pharmacy specialist at TMC provides pharmacotherapy support to medical staff and providers to optimize medication selection, dosing, and monitoring. Clinical pharmacy specialists participate multidisciplinary rounds and collaborations to optimize pharmacotherapeutic care of all applicable patients. The Clinical pharmacy specialist applies their specialized knowledge and clinical experience in evaluating the appropriateness and effectiveness of the patient’s medications. They serve as an evidence-based source of therapeutic information and recommendation. Clinical pharmacy specialists are an integral part of Antimicrobial Stewardship by ensuring appropriate antimicrobial therapy is utilized for the appropriate duration of therapy. The Clinical Pharmacy specialist contributes to new and evolving initiatives and participates in the education of nursing, medical staff, and pharmacy staff.
