As a Discharge Planner at our Martinsville, Indiana location, you will be responsible for coordinating and implementing effective discharge planning for patients across levels of care. This role collaborates closely with the treatment team, utilization review, patients, families, and community providers to ensure smooth transitions, continuity of care, and appropriate aftercare placement upon discharge.

Requirements

• Bachelor’s degree from an accredited college or university in a related field; applicable experience may be considered in lieu of a degree
• Three (3) to five (5) years of experience in clinical case management and/or discharge planning

Responsibilities

• Initiate and coordinate timely discharge planning for patients across all levels of care
• Collaborate with Utilization Review staff to support appropriate census management and maximize reimbursement
• Develop and maintain relationships with referral sources and community agencies serving individuals with substance use and co‑occurring mental health disorders
• Maintain updated referral source lists and contact information
• Provide written discharge plans to patients and families, identifying appropriate community resources and aftercare services
• Coordinate determination of appropriate levels of care for discharge placement
• Assess patient financial resources, including insurance benefits, and coordinate transitions to appropriate providers or referral sources
