The Care Coordinator assesses and coordinates home health, hospice, and infusion services for hospitalized, clinic, or outpatient patients, focusing primarily on coordinating infusion referrals for adult and pediatric patients and educating them on infusion devices. This role involves functioning as part of an interdisciplinary team, providing community education, and interfacing with referral sources and payors.
Candidates must possess a BSN and current RN licensure in North Carolina or a compact state, along with a minimum of three years of nursing experience. Basic Life Support (BLS) certification is required, though it may be renewed during orientation.

Team members work closely with a patient's physician to provide comprehensive, individualized care in the comfort of their home or at our inpatient hospice facility in Durham, NC.
Job Summary The Coordinator is a dynamic, fast paced position responsible for coordination of home health, hospice, and infusion services for adult and pediatric patients across the inpatient setting in the 3 Duke Hospitals. The primary focus is assessment of home needs and education of patients and caregivers related to the safe and effective use of home infusion devices, hospice and HH services. The coordinator works closely with the interdisciplinary teams collaborating with referral sources and community partners to ensure seamless, high‑quality care transitions. The ideal candidate has the ability to navigate complex care environments, build strong relationships across disciplines, and support seamless coordination between hospital systems and community‑based services.
