The role involves delivering compassionate, high-quality care to patients across the lifespan, partnering with providers for coordinated treatment, and supporting patients in understanding their health needs. Responsibilities also include accurate documentation in the EHR system and participation in quality improvement initiatives within a Patient-Centered Health Home model.
Candidates must possess an active LPN License or CMA Certification in good standing with North Carolina, be a graduate of an accredited program, and have a High School diploma/GED along with current CPR/BLS certification and a valid Driver's License. Preferred qualifications include one year of clinical experience in primary or community health settings and familiarity with underserved populations or PCMH models.
