Acts as a liaison between the community and healthcare, government, and social service systems to help individuals access essential resources. Provides support, informal counseling, and education to patients through both telephonic and in-person interactions.
Requires a high school diploma or equivalent and 0-3 years of experience. Candidates must possess basic computer skills and knowledge of medical and social community resources.

Community Health Specialists are also required to be available for community events that take place during nontraditional work hours (up to 25% of time).
