The Patient Navigator is responsible for documenting and tracking referrals using the EMR, obtaining pre-authorizations for services, and applying clinical knowledge to triage referrals based on urgency. They must also conduct pre-visit planning and communicate referral status to ensure care coordination and meet Patient-Centered Medical Home (PCMH) standards.
Candidates must possess a High School Diploma or equivalent, with a college degree preferred, and require two years of experience in a healthcare setting, with previous patient navigation experience preferred. Necessary skills include strong language and reasoning abilities, basic to intermediate computer proficiency (including EMR software), and the ability to travel between clinics.

We are a Federally Qualified Health Center (FQHC) that provides Article 16 and 28 clinic services at 8 locations across Suffolk County, New York. We are a multi-specialty healthcare provider caring for our community through Primary and Specialty Medical Care, Dental, Behavioral Health, Audiology, Physical Therapy, Occupational Therapy and Speech Language Pathology. We currently provide care to over 7,000 patients and have an annual operating revenue of $22m.
