We are a Federally Qualified Health Center (FQHC) that provides Article 16 and 28 clinic services at 8 locations across Suffolk County, New York. We are a multi-specialty healthcare provider caring for our community through Primary and Specialty Medical Care, Dental, Behavioral Health, Audiology, Physical Therapy, Occupational Therapy and Speech Language Pathology. About the Role

• As a Nurse Practitioner in Smithtown, NY, you will play a crucial role in delivering high-quality healthcare to patients across various age groups, including pediatrics and geriatrics.
• Your primary responsibility will be to conduct comprehensive clinical assessments, develop treatment plans, and manage pain effectively, ensuring patient safety and well-being.
• You will collaborate with a multidisciplinary team to provide holistic care, addressing both physical and emotional health needs.

Minimum Qualifications

• Master's degree in Nursing with a specialization as a Nurse Practitioner.
• Current Nurse Practitioner license in the state of New York.
• Certification in Family Medicine or a related specialty.

• Familiarity with electronic health record (EHR) systems.
• Bilingual skills to communicate effectively with diverse patient populations.
