The Physical Therapist will perform initial and ongoing evaluations, assessing and developing treatment plans with short and long-term goals to promote optimum function and safety. This role requires documenting progress and facilitating communication between staff, family, guardians, and medical providers.
Candidates must possess a valid Physical Therapist license in Minnesota and demonstrate excellent interpersonal skills for professional interaction with diverse individuals. A working knowledge of Microsoft Office is required, along with the ability to read, write, and speak English fluently.
Perform initial and ongoing physical therapy evaluations according to company policy and procedures. Assess, develop and implement a plan of care to promote optimum function and safety. Develop a treatment plan for each patient based on initial and ongoing evaluations including short and long-term goals, collaboration demands of other staff members which will require patient and caregiver involvement. Document patient progress according to approved medical documentation and facility documentation procedures. Facilitate communication and serves as a liaison between and among nursing and direct staff, family and/or guardians, and community medical providers and professional consultants in cooperation with operations management. QualificationsRequirements: Valid license as a Physical Therapist in the state of Minnesota. Excellent interpersonal skills, including the ability to work respectfully with a diverse range of individuals in a professional manner. Working knowledge of Microsoft Office suite. Ability to read, write and speak fluently in English. Must provide valid license, and proof of insurance. Prefer two-years prior home health experience EOE Employer, drug free work environment
