Case Manager - Low Intensity Care Coordinator

Case Manager - Low Intensity Care Coordinator (Houston County) This position is based at our Children's Outpatient Clinic in Dothan Monday-Friday 8am-5pm.

Responsibilities

• Interview and determine needs of adolescents/families within the Houston County area
• Assist consumers in accessing community services inside and outside of the SpectraCare System
• Implement service plans
• Service as a liaison between consumers of the program and other community agencies
• Maintain client records using an electronic clinical record system

Requirements

• Must have a BS degree in Psychology or related mental health field
• Must have a good driving record free from DUI/reckless driving for the past (3) years- will be subject to initial and ongoing MVR checks
• Must successfully complete case management training through Alabama DMH and receive certification of that training upon hire
