The Advanced Practice Provider (APP FNP/PA) functions as a member of a medical team, primarily responsible for providing comprehensive care and education to patients and their families under the guidance of the Associate CMO. This role also involves assisting families in identifying community resources, reducing barriers to care, and coordinating services for high-risk, multi-problem families.
Candidates must have completed an advanced educational program preparing a registered professional nurse for primary health care services, registered by the NYS Education Department or equivalent, or be a graduate of an AMA approved PA program. Required credentials include current NYS licensure, relevant specialty certification (e.g., ANA, NPAB), BLS certification, and a NYS Infection Control certificate.
Description The health services that became the Anthony L Jordan Health Corporation (Jordan Health) began more than 100 years ago, in 1904. As one of the first 5 Federally Qualified Health Centers (FQHC) established in the nation, its roots are steeped in service to those who face barriers to health and health care, meeting their need for comprehensive medical, dentistry, behavioral health, and community services. Starting in northeast Rochester, NY, Jordan Health has since expanded to become a network of primary care offices and health centers serving residents living in three quadrants of Rochester, NY and in Canandaigua, N.Y.
Jordan Health is now seeking an APP FNP/PA who, under the guidance of the Associate CMO, is primarily responsible for functioning as a member of a medical team to provide comprehensive care and education to patient and families. Assists families in identifying community resources, reducing barriers to access care utilizing support systems. Coordinates services for high-risk, multi-problem families.
