This position is responsible for leading and supporting colorectal cancer screening efforts. This role will serve as a clinical navigator, coordinating patient care, educating patients, and ensuring timely follow-up for colorectal cancer screening. The Navigator will work collaboratively with healthcare providers, patients, and community partners to reduce barriers to screening and improve outcomes. 1. Stays informed on current research and developments in colorectal cancer screening and treatment to provide the most relevant and effective education and support for patients, families, and health center staff. 2. Analyzes health center data to identify patients that are eligible for colorectal cancer screening. 3. Utilizes knowledge and expertise in healthcare and colorectal cancer to educate patients and their families on the importance of screening and early detection. 4. Guides patients through the colorectal cancer screening process, provide emotional support, answer questions, and address any concerns or anxieties they may have about the process. 5. Advocates for patients to ensure that they receive timely and appropriate care throughout their screening journey. 6. Collaborates with healthcare providers to coordinate and facilitate appointments, procedures, and follow-up care for patients. 7. Maintains accurate and up-to-date records of patient interactions, screenings, and outcomes. 8. Serves as a liaison between patients, healthcare providers, and other support services within the community to ensure a seamless and comprehensive patient screening experience. 9. Participates in health center and community outreach and education initiatives to increase awareness and promote the importance of colorectal cancer screening. 10. Continuously assesses and improves the patient navigation program to enhance the overall patient experience and outcomes. 11. Collects and maintains accuracy of all required grant data. Collaborates with grant partners and internal health center partners to prepare and disseminate data in a timely manner, as specified in the grant project. 12. Actively engages and communicates with all CRC screening grant partner learning initiatives and learning collaborative opportunities to include VACCP peer learning sessions and the Virginia Cancer Screening learning collaborative led by the University of Virginia. 13. Joins and maintains membership in the Cancer Action Coalition of Virginia (CACV) and participates in quarterly meetings and state cancer conferences. Joins CACVs VACRCRT and participates in their quarterly roundtable calls and meetings. Other Functions: 1. Physical Demand and Working Environment: Fast-paced office setting with travel to other offices often. Lifting and/or exerting force up to 15 pounds occasionally, with frequently moving of objects. Work requires speaking, sitting, bending, walking, standing, hearing, and stooping, kneeling, and repetitive motion with certain activities. Up to 8 hours of constant computer usage. Licensed Practical Nurse (LPN) or Registered Nurse (RN) with current Virgina license in good standing 2. Medical Assistant with Associates or higher-level degree in related field 3. Will consider individuals with bachelor’s degree in health sciences, public health, social work, or related field 4. Minimum of 2 years in a healthcare setting 5. Familiarity with appointment scheduling, patient education, and addressing barriers to care required 6. Experience in patient navigation, case management, or public health outreach preferred. 7. Experience working with individuals with difficulty accessing healthcare preferred
