The Patient Flow Facilitator-RN is responsible for facilitating patient flow, including transporting patients and expediting admissions, transfers, and discharges. The role requires functioning independently in a patient-centered care model and demonstrating advanced clinical practice.
Candidates must have a Bachelor of Science Degree in Nursing and a minimum of two years of clinical experience in pediatric intensive care is preferred. An active California RN license and certifications in BLS, ACLS, and PALS are required.
The Patient Flow Facilitator (PFF)-RN is a highly skilled licensed registered nurse that is competent to provide all levels of care (emergency, intensive, step-down, acute) to pediatric patients admitted to Loma Linda University Children's Hospital (LLUCH). The PFF nurse does not have a physical home unit, but instead, functions out of a home department. The PFF nurse's primary responsibility is to facilitate patient flow. This includes, but is not limited to, maintaining specified level of care while transporting patients with admission orders from the emergency department to the unit, transporting patients within LLUCH units, and assisting with expediting admissions, transfers, and discharges. Based on business needs, may be required to accept a full or partial patient assignment. Functions in a patient-centered care model of practice and performs the nursing process independently. Serves as an exemplary role model in patient care delivery methods and consistently demonstrates an advanced level of clinical practice, communication, and documentation. Facilitates the empowerment of the nursing profession and excels in implementing new knowledge, innovations and improvements in practice to enhance the patient experience and clinical outcomes. Applies theoretical concepts to clinical practice and continually expands knowledge base and clinical skills with a focus in the pediatric population. Utilizes interpersonal skills to maximize excellence in customer service, safe patient care, and professionalism. Contributes to a work environment of caring and cooperation among a culturally diverse workforce and patient population. May be required to participate in on-call rotation based on department business needs. Performs other duties as needed.
Bachelor of Science Degree in Nursing required. Minimum two years of clinical experience in pediatric intensive care preferred.
Active California Registered Nurse (RN) license required. Basic Life Support (BLS), Advanced Cardiac Life Support (ACLS) and Pediatric Advanced Life Support (PALS) certification through the American Heart Association required. Meets other requirements specified in the Plans for Providing Care for acute care, intermediate, and intensive care units as assigned.
