The role involves providing comprehensive, coordinated medical care, including acute and preventive services, to adults and children utilizing assessment and disease management skills. This includes performing basic procedures or referring patients as necessary while working collaboratively within a team structure.
Candidates must be graduates of an accredited physician assistant or nurse practitioner training program, certified and licensed in Family Practice by the CT State License. A minimum of one year of clinical work experience as an APRN or PA is required, with experience in urgent care or primary care settings being preferred.

Generally, the working conditions are good with little or no exposure to extremes in health, safety hazards and/or hazardous materials. This person must have the ability to travel as required to work with staff.
Education and/or Experience: Graduate of an accredited, nationally recognized physician assistant or nurse practitioner training program. Certified as a physician assistant or nurse practitioner in Family Practice. Licensed as either a physician assistant or family nurse practitioner with a CT State License. Experience in an urgent care or emergency room setting preferred. Minimum one-year clinical work experience as an APRN or PA required. Experience in a primary care setting preferred
Possess good interpersonal, multi-tasking, and organizational skills. Good judgment and decision–making skills. Computer literacy. Meets productivity guidelines as set forth by organization. CPR /BLS Certified Ability to work with a culturally diverse population. Bi-Lingual Spanish speaking a plus
