The Hospice Social Worker will support patients and families facing terminal illness by providing emotional support, coordinating resources, offering crisis intervention, and providing compassionate guidance aligned with holistic care principles. Essential functions include conducting psychosocial assessments, providing counseling, educating on advance directives, coordinating practical needs, and collaborating within the interdisciplinary team to develop individualized care plans.
Candidates must possess a Master’s degree in Social Work (MSW) and are preferred to have one year of social work experience in a hospice setting, demonstrating a strong understanding of hospice philosophy and end-of-life issues. Applicants must also be able to work tactfully with diverse groups and must have a licensed driver with an insured, reliable automobile if performing home visits.
