The RN provides trauma-informed nursing support and medication management for justice-involved residents with serious mental illness. They coordinate with a multidisciplinary team to integrate medical care into comprehensive treatment plans.
Requires a New York State Registered Professional Nurse license and current CPR/BLS certification. Preference is given to candidates with at least two years of experience in behavioral health or substance use treatment.
include but are not limited to: Conduct health assessments for all new resident admissions, including health screenings, medication review, and medical history documentation Review, reconcile, and monitor all resident medications in accordance with OMH and agency policy Oversee the program’s medication administration and monitoring procedures, including training residential staff as needed Maintain and review Medication Administration Records (MARs) to ensure accuracy and compliance Monitor residents for medication side effects, adverse reactions, and changes in physical health status Coordinate care with outside medical providers, the partnered Article 31 clinic, pharmacies, and hospitals to ensure continuity of care Provide health education to residents on medication adherence, hygiene, chronic illness management, and preventive care Assist in developing policies and procedures related to medical care, infection control, and medication management Participate in interdisciplinary team meetings, case conferences, and incident reviews as required Support regulatory compliance with OMH, DOH, and agency standards Maintain accurate and timely documentation in the electronic health record Provide consultation to residential staff regarding health-related concerns Upholding the mission, values, and trauma-informed care principles of Hope House and the Greenburger Center for Social and Criminal Justice, Inc. The RN is responsible for overseeing the medication system and medical coordination within the residence. Direct medication administration may occur as needed but is not required for all scheduled medication times.
Licensed Registered Professional Nurse in New York State Minimum two (2) years nursing experience preferred Experience in behavioral health, residential, community health, or substance use treatment preferred Experience working with individuals with Serious Mental Illness strongly preferred Knowledge of trauma-informed and recovery-oriented care principles Strong organizational, documentation, and assessment skills Ability to work collaboratively within a multidisciplinary team Experience with electronic health records preferred Current CPR/BLS certification required
\n $55,000 - $55,000 a year \n This is a part-time position averaging 20 hours per week, with a flexible schedule determined in collaboration with the Program Director to meet program needs. Some administrative or coordination duties may be completed remotely when appropriate. This position reports directly to the Program Director.
