Provide home-based care to patients with illnesses, injuries, and chronic diseases, often post-discharge from facilities. Responsibilities include examining patients, conducting assessments, documenting progress, reporting changes to physicians, and educating patients and families on care plans.
Candidates must be a Registered Nurse (RN) with an active, unrestricted license in the State of Michigan, and possess at least one year of clinical experience as an RN. A BSN is not required, but a current CPR Certification and reliable transportation with valid insurance and driver's license are mandatory.
