The nurse practitioner will conduct thorough health assessments, develop personalized care plans, and administer treatments as prescribed. They will also educate patients and families on health management and collaborate with other healthcare professionals.
Candidates must have a Master’s degree in nursing with a specialization as a Nurse Practitioner and current Ohio state licensure. Experience in home care or related fields, along with strong clinical and communication skills, is preferred.
PATIENT ASSESSMENT AND EVALUATION: Conduct through health assessments to identify patient need and develop personalized care plans. Regularly evaluate patient conditions and adjust treatment plans as necessary. CLINICAL CARE: Administer medications, treatments, and other therapeutic interventions as prescribed. Perform minor procedures and wound care as required. Monitor vital signs and patient response to treatment. PATIENT EDUCATION: Educate patients and their families on health management, disease prevention, and treatment options. Provide guidance on lifestyle modifications and self-care practices. COLLABORATION AND COMMUNICATION Work closely with physicians, nurses, and other healthcare professionals to coordinate comprehensive patient care. Maintain clear and concise communication with interdisciplinary team members. DOCUMENTATION: Accurately document patient assessments, care plans, and interventions in a timely manner. Ensure compliance with all relevant healthcare regulations and standards. QualificationsQUALIFICATIONS: EDUCATION AND LICENSURE Master’s degree in nursing with a specialization as a Nurse Practitioner in General Medicine or a related field of practice. Current and unrestricted Ohio state licensure as a Nurse Practitioner. Active and valid DEA license. Ability to participate in Medicare and Medicaid programs. EXPERIENCE Proven experience as a Nurse Practitioner, preferably in-home, in a Skilled Nursing Facility, Assisted Living, or a related area of expertise. Familiarity with geriatric and chronic care management. SKILLS Excellent clinical and diagnostic skills. Strong interpersonal and communication skills. Ability to work independently and as part of a collaborative team.
