• Position: Each patient that is referred to the Physical Therapy Program by a CTC physician will receive a complete evaluation by a licensed physical therapist. Each patient will then receive treatment from a licensed physical therapist and/or physical therapist assistant according to an individualized treatment plan. All services are to be recorded in the Electronic Health Record (EHR).

• Duties include, but are not limited to: Treat assigned patients according to plan established by supervising physical therapist Document each visit in the Electronic Health Record (EHR) within 24 hours of treatment Document and discuss progress toward established goals with supervising physical therapist Participate in team discussion surrounding patients’ plan of care, goals and continued therapy Communicate with supervising physical therapist in person or by phone
• QualificationsQualifications:
• Basic
• Knowledge and
• Skills: Understanding of substance and mental health issues required. Interpersonal and communication skills sufficient to effectively interact with patients, physicians, and health team members required. Computer proficiency inclusive of ability to access, enter, and interpret computerized data/information required. Basic Life Support certification required (BLS). Associate degree from accredited program required Arizona Physical Therapist Assistant license with ability to work under general supervision required Must be at least 21 years of age.
